To ensure your claim request is handled effectively and efficiently within the required timeframe, we kindly ask you to complete the following documentation, procedural paperwork, and other necessary requirements

Carrier’s Liability – Required Documents

  • Duly completed claim notification form
  • Police report
  • Contract of carriage
  • Dispatch Receipt (DR) (if required)
  • Good receiving voucher (GRV)
  • Calibration report (only for tanker)
  • Survey report
  • Invoice indicating the claimed indemnity amount
  • Valid driver’s license
  • Vehicle ownership/title certificate

Engineering – CPM (Contractors’ Plant & Machinery)- Required documents

  • Duly completed claim notification form
  • Police report (where applicable)
  • Valid operator’s driving license
  • Title deed/ownership certificate
  • The New Replacement Value (NRV) shall be treated as the sum insured
CPM and CAR Claims Notification Form

Fire and Lightning – Required Documents

  • Duly completed Claim Notification Form
  • Official Police Report
  • Fire Brigade Incident Report
  • Forensic Investigation Report
  • Proof of Ownership
  • Stock Declaration for the insured contents
  • Sales and Purchase Invoices related to the contents
  • Audit Report if applicable
  • Mortgagee Clearance Letter (where the property is subject to bank interest)

Marine Cargo - Required documents

  • Duly completed claim notification form or official letter from the insured
  • Original policy or certificate
  • Proforma invoice
  • Commercial Invoice
  • Packing list
  • Copy of bill of lading
  • Arrival notification
  • Letter of protest against the responsible party
  • Police report (if required)
  • Preliminary survey report (if required)
  • Short land certificate (if required)
  • Survey Report

Motor - Required Documents

  • The insured or his/her authorized representative must report the accident promptly through a written letter, email, telephone, telegram, or fax.
  • A duly completed claim notification form.
  • Original and copy of the valid driving license.
  • Original and copy of the vehicle title deed certificate.
  • Power of Attorney, where applicable.
  • An official police report related to the accident.
  • Proforma invoice covering labor charges and replacement parts.
  • Settlement of applicable excess and contribution amounts, if any.
  • Return of salvage items, where required.

Personal Accident - Required Documents

  • A duly completed claim notification form.
  • Police report, where applicable.
  • Medical documents, including sick leave, medical, and disability certificates.
  • Prescriptions for all purchased medications.

In the event of death:

  • Post-mortem examination certificate.
  • Court-issued confirmation document identifying legal heirs.
